Dental Lab Policy on Crown Remakes

Policy Statement: At Evodent Dental Lab, we are dedicated to delivering high-quality dental products and services. To ensure client satisfaction, we offer a remake policy for crowns fabricated in our office.

Remake Policy

Free Remakes:

We provide free remakes for crowns made in our office within 5 years of the original date of service, under the conditions outlined below.

Eligibility Criteria:

1. The remake request must be submitted within 5 years of the original crown being made.
2. The original crown must have been fabricated by our lab.
3. The dentist requesting the remake must be an active account holder with our lab at the time of the remake request.

Limitations:

This policy applies only to crowns made by our lab and does not cover other types of restorations or dental products.

The free remake policy is contingent upon the dentist maintaining an active account status with our lab. If the dentist’s account is inactive or has been terminated, the policy may not apply.
